To request accommodations, complete the preliminary survey and submit it to Student Engagement, along with supporting documentation (e.g., medical records, psycho-educational testing, or school records). If you don’t have access to these documents, meet with the disability access specialist to discuss other options.
We recommend scheduling an appointment before the semester starts to discuss accommodations. Research shows that early access to services leads to better outcomes.
