Immunization requirements

All new and transfer students must submit proof of the following vaccinations before finalizing course registration:

• Tetanus, diphtheria, and pertussis (Tdap): At least three doses total; one dose must be within the past 10 years.
• Measles, Mumps, and Rubella (MMR): At least two doses. Laboratory (serologic) proof of immunity is also accepted.
• Meningococcal Conjugate Vaccine (MCV4): Required only for students under 22 years old. One dose on or after age 16.

Tuberculosis screening

All international students and students returning from high-risk countries must complete TB screening. Submit documentation of one of the following:

• A PPD (Mantoux) skin test within the past year
• If you have a history of positive PPD or BCG vaccine:
  • Interferon Gamma Release Assay (IGRA) results,
  • or a chest X-ray report,
  • or proof of completed TB treatment

Uploading records or requesting an exemption

Use the secure portal (log-in required) to submit your Immunization Record Form or request a medical or religious exemption.

Note: Your form must be submitted before you can register for classes.

Student-athletes

If you’re participating on a North Park intercollegiate athletic team, you must also complete a Pre-Participation Packet.

Recommended vaccines

In addition to required immunizations, North Park recommends the following vaccines to help protect your health during college.

Meningitis B Vaccine

Meningococcal disease can cause life-threatening infections like meningitis, especially in college students living in dorms. Two types of vaccines are available: Meningococcal conjugate vaccine (protects against A, C, W, and Y) and Meningococcal serogroup B vaccine (protects against the most common U.S. strain in young adults)

Required: Students under 22 must have the conjugate vaccine.

Recommended: We encourage all students to get both vaccines.

Seasonal flu vaccine

The flu vaccine helps prevent illness during peak season (October–May). The CDC recommends getting vaccinated by the end of October.

Health Services offers flu shots each fall, often at reduced cost for uninsured students.

HPV vaccine

HPV is the most common STI and can cause several types of cancer, including cervical, throat, and anal cancer. The HPV vaccine prevents over 90% of these cases. It’s recommended for all college-age students.

Privacy and parental access

Health records are confidential. They cannot be released to parents or guardians without your written consent.

We encourage families to discuss health privacy laws, including the Family Educational Rights and Privacy Act (FERPA), before the school year begins.

Authorization for treatment of minors

If you’re under the age of 18, your parent or guardian must sign an Authorization for Treatment of Minors form. This form allows Health Services to provide care and contact your parent/guardian in a medical emergency. Return the signed form to Health Services in person or by fax.
